An Advance Medical Directive (AMD) is a legal document that outlines your preferences for care in case of a terminal illness. It ensures your medical treatment aligns with your values and takes the burden of decision-making off your loved ones, giving everyone peace of mind.
How Medical Directive (AMD) Works
The process is straightforward, and we’ll support you every step of the way:
Discuss with Family and Doctor Talk openly with your family and doctor about your decision to create an AMD. This ensures your wishes are understood and respected.
Complete the AMD Form Obtain the form from the Ministry of Health or your doctor. Fill it out with care, ensuring it reflects your wishes.
Witness the Signing Sign the form in the presence of two witnesses, one of whom must be a doctor. Both witnesses must be over 21 and not beneficiaries of your will.
Submit the Form Send the completed form by mail to the Registrar of Advance Medical Directives at the Ministry of Health. It cannot be submitted online.
After Registration Once registered, your AMD becomes a legal document, guiding your doctor to respect your preferences if you’re terminally ill and unable to decide for yourself.
Revoke or Update if Needed You can revoke or update your AMD anytime by completing a revocation form and sending it to the Ministry of Health, ensuring your choices always reflect your wishes.
